Benefits survey questions not populating

I've followed the instructions here, and am not completely new to questionnaires. I've attached the questionnaire under the self-service config as well as ensured that the Display Post-Enrollment Survey checkbox is checked. Still, when I go to enroll, I am not seeing my questions.

Any ideas as to what I might be missing?
