EOI certificate for Basic Life insurance not working

in Benefits

For Employee Basic Life plan, there is a requirement to set up EOI certificate for employees having coverage of more than 1 million USD with no interim (it should stop at 1 mil). It’s an automatic plan where the coverage is 1X of annual Compensation. Added the configuration as below, but it’s not working. Any suggestion as what is wrong?
