SQL Query to get the Variable profile Rate values to employees with benefit Plans

Summary:

My requirement is to pull the records from the from the Variable Rate Value which are associated with the Benefit plans.

we have two plan called Life Insurance based on the employee age they have configured some rate value for the retirees. I need pull it in the report.

Example:

Life Insurance (Buy Up)_Age 25 - 29 Rate Value 1.29

till the age of 90 they have configured few values. Let us know if you have any reports.
