Triggering notifications based on questionarre

Hi All,

One of our customer has a requirement where they have mentioned they have a journey declaration of interest where they use a questionnaire with three questions and one acknowledgement part. So there requirement is if in any of the questions there is a yes in the answer, it should trigger a notifications to the manager to review that part and take action and it should trigger separately for each of the questions if the answer is yes. After answering all the questions on submitting the acknowledgement it should again trigger a notifications to manager to review the DOI.
