Is it possible to support multiple printable performance forms per country? Or do they all have to share the common document from BI?

Is it possible to support multiple printable performance forms per country? Or do they all have to share the common document from BI? 

I have a requirement to fulfill that countries which use different performance template configuration need to utilize a different printable performance form. We have already been successful in modifying the two seeded printable forms (in regards to formatting and including a company logo):
