Goals not pulling into Performance Review

We are having some issues with employee goals not updating within the performance review document.  We can see that the goals exist but when we refresh / update the goals / competencies, the goals don't refresh.  In some cases, one but not all of the goals will refresh.  Overall, it is working fine but there are a few employees experiencing this.  Has anyone else had this issue.
