Capturing a response from a third party in Onboarding Task (Parent/Guardian form)

Hello,

We are in the process of reviewing one of our current Onboarding tasks to see if there is possibly an easier way to gather information from a third party. Currently, this is an Onboarding task where the new hire (if under age of 18) prints our parent/guardian acknowledgement form in Taleo through a content page. Once the parent signs the form, the new hire brings the form into the workplace on the first day.

We are looking to replace this task with an Onboarding task which will allow the new hire to enter their parent's email address, which will then send an email notification with the form to the parent's email. Has anyone had any experience with this type of task or have you come across any other options for this scenario within Taleo?
