Recruiter activity report

I am currently in the process of buidling what we are calling a Tally sheet which will capture Recruiter activity - see screenshot below.  Today our Recruiters are manually tracking this activity and we are trying to find a way to pull this out of OBI.

 

I have looked and don't see anything standard that will provide this - so reaching out to the community to see if someone can provide any insight.
