Participant Feedback Task Is Causing An ADF Error - After Nov Patch

We have included a Participant Feedback Section in our Performance document template. When a user clicks on the button to Begin the feedback process. It causes an application error and does not open the page. This error is consistent and we cannot get to the page. This is happening after the Nov patch. It worked before the patch.
