Performance Questionnaire

Hello,
I want to configure the set goal step with the possibility to configure a questionnaire but, doing this, the employee cannot see it when he opens the performance document. I suppose that the questionnaire must be added only during the manager/employee evaluation. Do you confirm? Have anybody ever added the questionnaire in other steps different from the evaluation one?
