Issue with Questionnaire template on Check-Ins

I am having an issue with not all of my questions showing on the Check-In TmpltContent
Good day Everyone,
I've created templates successfully so far. However, I am having an issue with my questionnaire templates that I attach to the Check-Ins. All of my questions are not appearing in the Check-In.
I've configured the questions with out issue... I can see my questions on my templates but when attached to the check-ins not all of of the questions show up.
Has this happen to anyone else, if so how did you resolve?
