Employee Cannot See Performance Document

We are a month into our 2018 performance cycle, and we have had reports from a few employees that they cannot see their performance document at all (see screenshot below for example). I cannot see anything wrong with their data. They definitely have the document assigned to them (verified on the Determine Eligibility page). But they cannot see the document, and their name and job title don't even show under the Filter icon.
Has anyone ever experienced this? What could cause it? We are at a complete loss as to what is going on.
