Comments not saving in employee performance document

Some employees are not able to save their comments when completing their performance documentContent
We have an employee who is unable to save comments she entered in her performance document. the employee clicked 'view all comments' in the competencies section of the performance document. The employee typed comments in the comment section. when the employee clicked save, the comments disappeared. the employee has tried saving comments using different browsers with the same results.
with that said, only one employee has reported this issue. Other employees are able to save comments in their performance documents.
Has anyone experienced this issue?
