Performance Evaluation - First Approval Task - Not Initiating

Dear All,

We have issue with few employee's performance evaluation process.

When the line manager Initiates the first Approval Task , the Task is not getting Initiated. Instead the Task is returning back to the Line Manager for Initiating the "First Approval" Task.

Any Idea , how to troubleshoot this issue
