Matrix manager as participant

Hi

I have read through a bunch of the posts concerning this topic.   We have the following requirement:

- The Manager's manager is an active participant in the performance evaluation process and will a) make comments and b)may even change the manager's ratings.  

Do I need to set up a questionairre for this or can the manager just select his manager as a participant and then have the manager's manager update the performance document with his comments ?
