Enable two types of responses for a one question

We have a requirement to conduct 360 feedback survey using performance management. Once user doing this survey every question ( behaviors of the individual)  has two rating scales. One is to check how often use this behaviors and the other scale is how effectively perform this behavior

 

ex :

                                                                                   Frequency                       Effectiveness

Build Sustainable Relationship with customers            1                                        2

 

In the preview the question should appearing like above mentioned way.

So when setting responses for a question we can set only one rating model or when giving responses with out giving the rating model, 

Is there a way to achieve this?
