Setup Request: Benefit premium multiplied by percentage depending on EEs salary range

Looking for setup to fulfill requirement of monthly premiums being multipled by a percentage based on the employees salary range. Please see below example.Content

Example: Employee enrolled in Employee + Spouse that makes $42,000/year = 1480* 8% = $118.40
Question: I know we will have to do the below, but how do we get the premium to multiply by the percentage amount (8%) based on the salary range?
