Setup Request: Benefit premium multiplied by percentage depending on EEs salary range

Summary

Looking for setup to fulfill requirement of monthly premiums being multipled by a percentage based on the employees salary range. Please see below example.

less than $25,000 4%
25 - 29,999.99 5%
30 - 34,999.99 6%
35 - 39,999.99 7%
40 - 44.999.99 8%

Example: Employee enrolled in Employee + Spouse that makes $42,000/year = 1480* 8% = $118.40

Question: I know we will have to do the below, but how do we get the premium to multiply by the percentage amount (8%) based on the salary range?

  1. Derived Factor based on Salary ranges
  2. Eligibility profiles based on Salary Ranges
  3. Variable rate for each salary range using calculation method multiple of compensation
