Performance Reviews additional functionality

Hi Everyone,

We have a request from the business to build out within the Performance review, a place where they can ask the manager to fill out a "Next Career Move Readiness" and a "Target Promotion Date".  We already have an existing questionnaire for the Former/Additional Manager and we have used all the content sections.  From my understanding, you can only have 1 questionnaire and you cannot add to the content sections.  Not sure if there is a solution for this, but wanted to reach out.

In addition, we only want this to be viewed by the managers.  

We are currently on 19A and will have 19B in 2 weeks.  We do not have Newsfeed or Mobile responsive pages enabled yet.
