Performance Document Name Not Coming Up In The Dropdown While Running The Eligibility Batch Process

While running the eligibility batch process under performance tab, the performance template is not showing in the drop down menu, it is blank.

Read Doc ID 2110497.1 and ensured eligibility profile was added to document period.  

How do i get my performance document to show?
