Incidents are currently pulling in multiple times due to multiple thread comments per incident

I am trying to create a report that shows a count of incidents per Root Cause Category level. The incidents are pulling in multiple items per each incident due to multiple thread comments on each incident. Please help provide any suggestions in hiding the duplicate line items and just counting the first thread comment on each.
