Adding DFF Values to a Questionnaire in Performance Evaluation

There is a requirement to capture the answer for question when evaluating a worker.

If a questionnaire section is added, is it able to add a DFF to the questionnaire attached to the performance document to capture the value (answer) to get it to calculate the overall ratings for a worker. Because in this case, the final performance ratings need to get using a formula and if a formula is used there must be way to capture the answer for the question. Is there any idea on this ?
