Email notification Approve/Reject buttons not working as expected

Content

Implimented actionable notification for approvals. Approver received email notification with Appove/Reject buttons.

When user clicked on the approve/Reject button, new email pop up and hit send.

Nothing happend. After 2 min received the following message.

Your response to this notification was not processed.

Retry sending email later. You can also login to worklist application to act on pending tasks. .

Please advise what I am missing.
