Printing Questionnaire Section Headers

Hello,
I've recently created a Questionnaire that has multiple sections, each having 4-5 questions each. On the screen in the performance review, the section headers are visible and clearly delineate the different areas. However, those section header disappear when the performance document is printed, which makes it harder to read and undermines the reason we used a questionnaire in the first place. Am I missing a setting somewhere or is this by design?
I've attached screen shots showing the view from the Performance area versus the printed version.
