Unable to setup sub competencies

Content

We are trying to setup competencies and sub competencies for a group of employees. What we want is in the attachment. I tried using behaviors and linking them to competencies, but, it did not work and I gave up. I also tried "Questionnaire". Questionnaire did not even show up when Performance Document was created. Any ideas, please?
