Goals Disappearing from Goal Plan

Summary

We've been successfully adding goals to a goal plan and then creating performance documents, but upon returning to the goal plan after a few days/weeks, we notice the goal plan is empty.

We have been using dummy employees as testers for the last few weeks. With these employees, we added many goals to their goal plans and then created performance docs which populated with the worker goals. All of this was successful and passed. This week as I looked back at some of the old test user profiles, I noticed that their goals are now missing from their goal plan. We did not delete these goals, but they are not presented to the user in the UI.
