This has been an exceedingly frequent occurrence in triage of approvals related issues. 

Problem:

The customer sees multiple approval requests for the same requisition being sent to approvers

The customer sees Document Action History indicating that the System Administrator has withdrawn an approval notification

The customer sees one approval process open even though the requisition has been notification

Analysis:

All the above symptoms commonly lead to one issue in a majority of the cases - The requester that submitted the requisition withdrew said requisition almost immediately and then resubmitted the same requisition.
