Unable to view Check-In template

Hi,

I have created Check- in template with details :

From - To Date  : 1/1/2020 - 12/31/2020

Include in Performance Document : Checked

Questionnaire : selected for worker and manager same.

Eligibility - None

 

When I navigate to add check - in template, under career and performance, I can see review period although no add button to add the above mentioned check-in temp its just blank. If its based on review period where do I integrate these two ?

I assumed I don't need to run the eligibility process as I none has added in the template.

I did check the role/ privilege in both manager and employee role and all required ones are present namely
