The approval transaction failed because of an error.

An employee is receiving the following error when trying to send his goals to his manager for approval. "The approval transaction failed because of an error. Submit goal changes for approval again. If the problem persists, contact your HR specialist. (HRG-3120298)

Has anyone else had this issue and how did you overcome it? We have just implement Goals & Performances so are on V13
