Attachment and new mandatory field on the Goal Section

Dear Experts,
We need to add an attachment area and a mandatory free text field in the Goal section for each goal. I am sharing the screenshot for more clarity. Kindly let us know the possibilities.
The page is visible when you click on the Goal section and rate all the goals(whether by employee or manager). We want to have an attachment area and a free text field for each goal.
