How many times severity updated for a incident

Hi,

We have business requirement to get number of times severity got updated for one particular incident and who updated it.

For example -

Incident ID                Number of times Severity Updated                 Who Updated

123                                   3                                                                Person A, Person B, Person C

3456                                 2                                                                Person A, Person C

 

I got one standard report ID - 519 (Open Incidents By Severity) but not useful. Any one has an idea how to get similar kind of result.

Is there any standard or custom report which shows number of times a particular field got updated.
