Performance document questionnaire in a separate section but appears under Evaluation Topics

Hello,

We have the below sections in our Year End Performance template

1. Overall Summary
2. Performance Goals
3. Development Goals
4. Questionnaire

But the document shows the Performance Goals, Development Goals and Questionnaire under the Evaluation Topics section.(see enclosed image)

How do we have the Questionnaire appear in a separate section?

Also how do we remove the Evaluate button from Development and Performance Objectives?

I was able to hide the Evaluate button for the two objectives sections using Page Composer.

Now looking at moving the Questionnaire to a separate section. Even though it is defined as a separate template section, the document groups it under Evaluation Topics. Any thoughts other than trying to use Page Composer?
