Employee Mid Year document details(Goals and Questionnaire) is getting Overridden

For couple of employees we observed that Comments entered in Goals and Questionnaire for one employees in mid year document is getting reflected in another employee Goals and questionnaire section 

 

Any idea why it is happening ?
