How to include SR object fields into Milestone object in Email templates?

Hi Team,

I have created an email template for SR escalation which will be trigger through Object workflow on Milestone object. In that template doesn't have Service requests fields like (Reference number,status,severity,etc..). So while escalation, we don't know for which SR the escalation was triggered.

Please refer the image and let me know how to add SR fields in to the Milestone object.
