EFF for Job Applications flow not showing

We have created an EFF to capture additional information from Candidates, we can see the EFF as a recruiting in both sections "Application Extra Info" and Extra Internal Info", however, we are just seeing the section name when we apply as a candidate but not the fields. 

Is there something we are missing?
