Is it possible to extract from Fast Formula the Performance Rating field by performance document

Hello experts,

we need to implement a compensation plan with custom columns displaying the Performance Rating field overtime (by performance document).

Is it possible to achieve this by Fast Formula?

Please see the attached screen. In other words, the fast formula should exctract Performance rating 4 and 5 according the performance document name. Is it possible to achieve this?
