From Hired through Probation to Annual Appraisal?

Dear Expert, how do you ensure workers are assigned the correct performance documents at the appropriate time? For example, how a newly hired person is assigned a probation performance document and then once probation has been passed, to be assigned the annual review performance document?  What is an efficient way to do this and what steps do you recommend??
