Is it possible to have a Questionnaire for the worker, without including the Worker Self Evaluation

Can we include a Questionnaire for workers to complete, without also including the Worker Self Evaluation task?

As the first step in the Performance Management process we would like the worker to complete a questionnaire, however this is the only step we have for Worker... After this, Manager completes the evaluation for goals/competencies/overall summary and also a questionnaire.

Is this possible?  Have tested and without Worker Self-Evaluation task, the worker can 'View' the questionnaire, but not 'Evaluate'..

The process flow I tried only has: 

1. Manager Evaluation of Worker

2. Share Performance Document
