Questionnaire Template In Use but I have not yet used it??

I am creating a number of questionnaire templates and questionnaires at the moment and am periodically coming across a problem when a Questionnaire Template is shown as 'In Use' when I have not created any Questionnaires with it yet.  I am not able to edit this questionnaire template or delete it, its so frustrating.  This has happened on a couple of occasions now and I havent done anything different. 

 

Does anybody have any ideas?
