In the past when I've had a situation where an employer allows an employee to choose whether to pay for benefits on either a pre- or post-tax basis, I've set up separate pre-tax and post-tax medical plans (and reused the same options) or set up one medical plan with separate pre-tax/post-tax options. This was also a bit easier to handle in the classic UI because of the ability of the employee to filter.
Currently, I have a situation in which I have 8 medical plans so how I've previously handled will be a bit clunky (either 16 medical plans with 4 options or 8 plans with 8 options).
