No add button under Development goals.

We're receiving a lot of issues about no add button under Approve Development Goals. When I tried to check the name of employee in transaction console under Approve Development Goals process I found out that there is stuck in Draft status and once I terminated this transaction add button in development goal is now able to access by the employee. Any idea what is the causing of this issue? Why this transaction went to draft status and getting stuck in console?
