Reuse question in questionnaire

Hello!

Is there anyway to reuse a comment follow up question for each question.  Client requirement for review is to have the manager and employee provide a rating and then a comment for each item being evaluated.  In some cases there are 40 questions each requiring the worker and manager to provide comments for each question.  

Also is it possible to merge the info instead of having two separate docs?  similar to the image attached?
