Issues with Request Information Notification

Submit button is greyed out and not allowing candidates to finalize.

We have been experiencing lately an influx of candidates not being able to click Submit on the Request Information Notification. The Contact Info section appears to be missing. I believe it may be a browser issue, but when they switch to a new (supported) browser the same thing happens. Causing the Submit button to grey out, and them not be able to complete the request for sensitive info. 

Has anyone else experienced this lately?
