How to populate increment value from a UDT to the Worksheet Display ?

Our client has a requirement where they do not wish to change the Dynamic values every year and would want to just load the values to a UDT and we should populate the increment percentage based on that.
So we have 3 columns
Performance Ratings | Grade | Increment |
5 | M1 | 10 |
4 | M1 | 8 |
4 | M2 | 9 |
Is there a way to pull the values of UDT through a fast formula? My developer was telling that it only works where there is only one criteria,eg Grade but when we try to pull a combination of Performance Ratings & Grade,its unable to pull the increment percentages.
Please let us know if we have a solution for this as there is a possibility that we might have to give 3 keys too based on client requirements
