Employee ER rate

Employee has Medical Insurance as EPO Employee + Spouse, however, there is no ER rate listed in her plan. It is at 0.00 dollars which is not right. There is an Employer Rate (ER) that should be listed to show how much the Employer owes each paycheck but this is not listed in Oracle. 

There should be an amount in there. I do not know the amount as this is something is generally automatically entered in Oracle when an employee elects their benefits. Please assist.
