Requisition is created through Punch out and it is Approved but PO is not created

We have a Requisition is created through Punch-out and it is also approved but PO is not created. Checked in the process requisition and document builder but unable to find it. Before asking requester to withdraw and re-submit the requisition 

Did anyone faced this issue?
