Can we embed a deep link in a performance document to provide Feedback?

We have a requirement wherein the Client wants a deep link for Feedback Templates to be embedded in a Performance Document so that whenever Manager is evaluating or Worker is completing the document, the document is in progress and they can access the link and provide feedback.
