BPM object workflow always getting set to Rejected approval status

Hi All,

For one of my customer requirement, we have created a BPM object workflow and set the properties as below yet the object workflow is always getting entered in to REJECTED status . No email is generated either.

Created a business process as below and deployed under BPM screen

Created an object workflow

Set below property under business process

set the approvers email field will store the user name of the approver and ApprovaStatus_c will store the status.
