Report Life event not working

Summary:

Not move to next Page when click on Continue under the function "Report Life Event".

Content (required):

We have defined Option and configured enrollment as Un-restricted and all employees are eligible. However, also defined rate by attaching payroll element.

But when employee login through self-service and report life event then unable to move or process, Actually screen got stuck at same place.

is there any other things need to be done.
