Optional Questions in Performance Review

Hello,
Looking for some ideas on how to manage 'optional' performance questions. Requirement: there is a standard questionnaire, and then a manager is allowed to add from a standard set of 10 questions if they want. They don't have to add any if they do not want. But if they do, they should be added to the score. They shouldn't be able to create the questions, they have to choose from the Library.
