Performance Process with No Ratings

Summary: I have been asked by senior management to create a performance process that does not contain any Ratings. Goals and Competencies should be available in the evaluation with only single comment box. There should not be individual Goals & Competencies comment box for Employees and Managers. There should be only one comment box for Goals and one for Competencies.

Is this possible or any alternative like default Rating which will be visible while evaluation & PD.
